Client Documentation - Health & Nutrition

** PLEASE READ THE FOLLOWING INFORMATION THOROUGHLY & CAREFULLY **

 

Hi there,

 

Thank you for you enquiry or booking an appointment session.

Attached is the paperwork (as a single PDF file) for you to download, print, complete, then scan and email (info@DrRonacher.com) the completed paperwork to me at least 24-48 hrs prior to the scheduled appointment session, and/or bring with you to your appointment session.

 

Please print and start to complete the paperwork ASAP to allow sufficient time to complete it fully.

 

During the initial Wholebody Health consultation I will:

 

   (1)  Review the completed paperwork and assess your current status of health (or ill-health);

  

   (2)  Address questions that you may have;

  

   (3)  Set the foundations/guidelines for improved health and wellbeing;

  

   (4a)  Put forward recommendations/action steps for you to implement to start to remedy your current presentations;

  

  AND/OR

  

   (4b)  Collect the necessary and vital information in order to formulate and prepare a personalised and comprehensive Wholebody Health Coaching & Mentoring Program that will be delivered to you in a systematic manner

 

I do offer several different service/support options and programs to clients to help them achieve their specific health and wellbeing goals, and as such we can discuss and evaluate the best option for you moving forward during your initial appointment session.

 

** My main focus is to educate, support and guide you to help you achieve your desired results. I use a wholefood nutritional therapy approach (i.e. foods as medicine) to assist you - and only recommend supplements, nutritional aids, etc. when necessary **

Please print out the attached PDF file, complete each document as thoroughly and as honestly as possible, then scan and email them to info@DrRonacher.com, at least 24-48 hours prior to the scheduled appointment session, and / or bring with you to your appointment session.


In relation to the Food & Health Diary it will only need to be maintained for 3-5 days, plus 1-2 'typical/average' days can also be completed if the diet, etc. during the 3-5 day recording period is different to the norm. It can also be back filled for previous days, if needed. Please record everything that you eat and drink. Be as thorough and detailed as possible. Please be as honest as you can be, I am just gathering data. Don’t change your normal routine, just record what you are doing. This diary is not used as a way of judging/critiquing you, but simply a means to get an understanding of the foods or dietary/lifestyle pattens that may not be supportive of your system and are causing you imbalances. Medications and/or supplements are to be recorded in the register document even if only taken/used on an 'as needs' basis - please make a note of these. In the Health Questionnaire please tick all the relevant current/recent presentations, also make a note(s) of any known family history of health challenges/dis-seases, i.e. cancer, heart disease, obesity, diabetes, asthma, etc. The more information that you can provide to me the better I can assist you - this includes self written summaries, blood test results, and/or doctor/specialist reports, etc.

NOTE:  A reminder text and/or email will be sent at least 24-48 hours prior to your appointment to confirm your scheduled time and as this appointment time has been specifically allocated for you.

 

Cancellation Policy:  We kindly ask that 24 hours notice be given where possible to cancel or reschedule your appointment, as this allows other clients ample time to fill the vacancy.  If you cancel your appointment within 24 hours of your scheduled time, or you do not attend your appointment, you will be charged 50% of the consultation fee. This fee can be paid at the time of your next appointment, or an invoice emailed/mailed to you directly.

 

Payment methods include: Cash, Bank Transfer (EFT), or EFTPOS (Savings / Cheque), Credit Card (VISA / MasterCard). Please note that EFTPOS & Credit Card payments incur a 1.9 to 2.2% processing fee (dependant on card type and processing method used).
